Drive timely hospice referrals and eligibility workups while building hospital relationships to become the hospice of choice.

  • Must-have: Texas RN or LMSW with healthcare marketing experience.
  • Build hospital partnerships to increase referrals and establish the hospice of choice.

Job Description

Responsibilities

  • Respond to referral sources in a timely manner; evaluate patient appropriateness for hospice; meet with families/patients to discuss goals and explain the services of hospice.
  • Complete required paperwork with patients/families; facilitate the patient discharge by working with hospital staff, DME, medications, hospice physicians, and hospital staff.
  • Educate hospital staff and follow up on pending patients; round daily to be available to staff and patients for questions and to build relationships.
  • Communicate and provide daily feedback to the admission staff on referrals.
  • Completes thorough documentation of hospice eligibility in the required time frame.
  • Through customer service and leveraging relationships, become the “hospice of choice” by increasing hospital referrals.
  • Attend and participate in routine staff meetings in person or via media provided as directed.
  • Participate in and/or provide in-service training, attend various hospital activities, and perform other duties as required or assigned

Requirements

  • Registered RN in the State of Texas, or LMSW required
  • Nursing experience or medical training required (ability to read charts, know medications, etc.)
  • Marketing experience required
  • 1-3 years of experience in healthcare marketing preferred, Hospice experience strongly preferred
  • Proficient skills using a computer are required
