Job Summary

Own hospice BD and community liaison work: build referral networks and formal contracts, educate providers about Moments hospice, and coordinate with BD and clinical leadership to grow partnerships while resolving client problems.

  • Growth & Partnerships: Develop new referral sources and contractual relationships with physicians, organizations, and payers.
  • Provider Education & Collaboration: Become the go-to expert on Moments hospice services and communicate effectively with providers.
  • Operations & Problem Resolution: Ensure new business initiation with office support and act as liaison to clients to resolve issues.

Job Description

Responsibilities

  • As a Clinical Nurse Liaison you will develop and implement programs to raise awareness of hospice services and establish professional relationships within the community by conducting personalized visitations to all primary referral sources and interested parties, and by representing the agency at community events.
  • Responsible to generate business from new referral sources, identify opportunities for formal/contractual relationships with physicians, organizations and payers.
  • Works with the office staff to ensure that new business is successfully initiated.
  • Become an expert regarding services provided by Moments hospice ensuring effective communication and collaboration with providers.
  • Work in partnership with BD and clinical leadership to develop business plan strategy.
  • Act as company liaison to clients to resolve problems.

Requirements

  • Must have valid RN license
  • Ability to work independently with minimal supervision.
  • Must have great clinical skills and be great communicator.
  • Ability to resolve problems and set priorities.
  • Excellent written and verbal skills to effectively communicate needs to the public and to field and office staff.
  • Pleasant telephone demeanor
  • Ability to work cooperatively with others, outgoing and personable.
  • Reliable transportation and valid driver's license and auto liability insurance
  • Successful completion of background check process

Preferred

  • Hospice Admissions experience strongly preferred
  • Previous Healthcare Marketing Medical Experience Preferred.
  • Hospice knowledge and experience preferred.
