Job Summary

Own the frontline family outreach and enrollment pipeline, coordinating with centers and partners to connect diverse families with preventive services and resources while maintaining confidentiality and trauma-informed care.

Family engagement and enrollment: Build trust, execute multi-channel outreach, and assess needs to enroll families in services.
Partnership coordination: Collaborate with center staff and community partners, and maintain accurate records and data entry.
Care delivery and access: Preserve confidentiality, uphold professional standards, and offer flexible hours to support families and program goals.

Job Description

Responsibilities

Building trust with families and connecting them to preventative services and community resources
Conducting outreach through phone, text, email, mail, and in-person engagement
Assessing family needs and supporting successful service enrollment
Collaborating with Family Resource Center staff, CMTs, and community partners
Tracking outreach efforts, maintaining accurate documentation, and managing data entry
Supporting trauma-informed, family-centered, and culturally responsive services
Maintaining confidentiality, professionalism, and positive community relationships
Flexibility to work evenings and occasional weekends as needed to support families and program goals
