Job Summary
Own the frontline family outreach and enrollment pipeline, coordinating with centers and partners to connect diverse families with preventive services and resources while maintaining confidentiality and trauma-informed care.
- Family engagement and enrollment: Build trust, execute multi-channel outreach, and assess needs to enroll families in services.
- Partnership coordination: Collaborate with center staff and community partners, and maintain accurate records and data entry.
- Care delivery and access: Preserve confidentiality, uphold professional standards, and offer flexible hours to support families and program goals.
Job Description
Responsibilities
- Building trust with families and connecting them to preventative services and community resources
- Conducting outreach through phone, text, email, mail, and in-person engagement
- Assessing family needs and supporting successful service enrollment
- Collaborating with Family Resource Center staff, CMTs, and community partners
- Tracking outreach efforts, maintaining accurate documentation, and managing data entry
- Supporting trauma-informed, family-centered, and culturally responsive services
- Maintaining confidentiality, professionalism, and positive community relationships
- Flexibility to work evenings and occasional weekends as needed to support families and program goals
