Job Summary

Coordinate hospital discharge referrals across San Diego County to reduce readmissions and streamline patient placement.

  • Develop and manage hospital referral relationships and discharge coordination.
  • Must-have: Degree in Social Work, Healthcare Admin, Public Health, or related field with existing San Diego hospital contacts.

Job Description

Responsibilities

  • Develop referral relationships with hospitals, health systems, and discharge planning departments
  • Conduct in-person outreach to hospitals, skilled nursing facilities, behavioral health programs, and community clinics
  • Maintain strong working relationships with case managers, social workers, and Molina Medi-Cal care coordinators
  • Educate healthcare partners on recuperative care placement options and admission criteria
  • Coordinate referrals and admissions with internal team members
  • Track referral activity and maintain communication with referring partners
  • Attend community healthcare networking events and outreach meetings

Requirements

  • Education: degree in Social Work, Healthcare Administration, Public Health, Nursing, or related field preferred
  • Experience working in hospital discharge planning, case management, medical social work, or healthcare liaison roles
  • Existing relationships with San Diego hospitals or healthcare systems
  • Knowledge of Medi-Cal, managed care plans
  • Experience with homeless services, behavioral health, or post-acute care placements
  • Strong communication and relationship-building skills
