Responsibilities

• Hospital & Referral Outreach: Build and maintain strong relationships with hospital discharge planners, case managers, social workers, and physicians to generate referrals.
• Hospital visits and coordination: Conduct regular hospital visits, attend discharge planning meetings, and coordinate with healthcare professionals to ensure smooth patient transitions.
• Opportunity tracking: Identify new business opportunities and track referral trends within the region.
• Community Engagement & Marketing: Represent the facility at community events, health fairs, senior centers, and local organizations.
• Marketing initiatives: Plan and execute community-based marketing initiatives and sponsorships that promote the facility’s programs and services.
• Brand-aligned materials: Develop and distribute promotional materials that align with the facility’s branding and compliance standards.
• Admissions Support: Partner with admissions and clinical teams to evaluate referrals and coordinate pre-admission processes.
• Data-driven decisions: Track, analyze, and report referral data and admission outcomes to guide marketing strategies.
• Brand & Relationship Management: Serve as the primary liaison between the facility and external partners.
• Program knowledge & market monitoring: Maintain up-to-date knowledge of the facility’s specialized services and rehabilitation programs; monitor competitors and market trends to identify growth opportunities.
