Job Summary

Own end-to-end admissions operations, driving referrals, conversions, and smooth transitions from inquiry to admission across the facility while maintaining budget and payer-mix targets.

  • Lead the admissions process from inquiry through discharge planning.
  • Grow and maintain external referral networks with hospitals, physicians, and case managers.
  • Ensure accurate financial verification and a seamless resident onboarding experience.

Job Description

Responsibilities

  • Report directly to the Facility Administrator.
  • Achieve targeted admission and referral goals through the execution of the center's marketing action plan.
  • Meet or exceed budgeted census and payer-mix goals.
  • Obtain and review referral information from telephone and walk-in inquiries, including all clinical and financial information required for admission committee approval.
  • Ensure financial verification is accurate and complete before admission to help reduce accounts receivable issues.
  • Obtain family commitment to tour the facility for all viable referrals.
  • Conduct professional facility tours and collaborate with clinical staff to address questions and encourage admission decisions.
  • Direct, educate, and coach facility staff on compliance with the admissions process.
  • Manage the admission process, including reviewing and explaining admission policies to residents and their families.
  • Ensure all admission paperwork is complete and that residents experience a smooth transition into the facility.

Requirements

  • 2–3 years of experience managing admissions in a LTC facility, subacute rehabilitation center, or other healthcare setting.
  • Demonstrated success developing referrals from healthcare target markets, including hospitals, physicians, case managers, and other referral partners.
  • Strong organizational skills with exceptional attention to detail, documentation, and follow-up.
  • Excellent professional telephone communication skills with the ability to effectively and efficiently manage referral calls.
  • Experience working directly with residents, families, referral sources, and healthcare professionals.
  • Strong interpersonal and relationship-building skills with the ability to conduct facility tours and effectively communicate the value of the facility's services.
  • Inside or outside sales experience preferred.
  • Proficiency with computers and marketing or customer relationship management systems.
  • Knowledge of Medicare and Medicaid guidelines and healthcare reimbursement processes.
  • Ability to work in a fast-paced, census-driven environment while meeting admission and referral goals.
