Job Summary

Own the referral network and admissions flow for hospice services by partnering with hospitals and care facilities, connecting with new sources, and aligning patient placement with the physician plan of care.

  • Referral Development: Build and maintain relationships with hospitals and health care sources to drive admissions.
  • Source Coordination: Serve as primary contact for referrals and ensure timely, accurate routing and intake.
  • Care Alignment: Identify appropriate services per plan of care and policy, supporting families through the transition.

Job Description

Responsibilities

  • Collaborate with the Hospice Liaison team and Admissions team to increase public awareness of hospice and other services and drive admissions for patient placements.
  • Act as the first point of contact for hospitals, health care facilities, and other sources regarding referrals.
  • Seek networking opportunities to educate and develop relationships with new and existing referral sources.
  • Identify appropriate care and services for patients and families in accordance with the attending physician and plan of care, as well as company policy and procedures.

Requirements

  • Bachelor’s degree in marketing, business administration, or other related fields is preferred. An acceptable combination of experience and/or training may be considered instead of formal education.
  • Licensed as a registered nurse in the state of practice, a plus
  • Must possess the ability to make independent decisions when circumstances warrant
  • Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and general public
  • Must be knowledgeable of quality assessment and assurance procedures
  • Valid driver’s license and proof of insurance is required

Preferred

  • Bachelor’s degree in marketing, business administration, or other related fields is preferred
