Job Description

Responsibilities
  • Hospital Liaison & Clinical Assessments: Conduct daily onsite visits to key referral hospitals. Review clinical charts, perform bedside assessments, and determine the appropriateness of referrals for admission to Whitney or Grandview.
  • Relationship Management: Build and maintain strong, productive partnerships with Hospital Case Managers, Discharge Planners, Physicians, and Social Workers to ensure a steady stream of high-quality referrals.
  • Census Development: Execute strategic marketing plans to meet and exceed monthly admission and occupancy goals for both buildings.
  • Admissions Support: Act as the primary backup and strategic partner to the Admissions Director at Grandview Healthcare Center, assisting with the internal admissions process, insurance verification, and family tours as needed.
  • Community Outreach: Represent the facilities at community events, networking groups, and professional seminars to increase brand awareness.
  • Data Reporting: Track and report on referral trends, conversion rates, and competitor activity to the Regional Operations team.
Requirements
  • Experience: Minimum of 3–5 years of experience in healthcare marketing or admissions, specifically within the Skilled Nursing Facility (SNF) or Long-Term Care (LTC) industry.
  • Clinical Knowledge: Strong ability to read and interpret medical records; a clinical background is a plus but not required.
  • Communication: Exceptional interpersonal skills with the ability to "sell" our clinical capabilities to hospital partners and families in crisis.
  • Technical Skills: Proficiency in CRM software and the Microsoft Office Suite (Excel, Word, PowerPoint).
  • Mobility: Must have a valid driver’s license and the ability to travel daily between hospitals and the two facilities.
