Job Summary

Own hospice referrals in Albuquerque through in-person outreach and education to build a broad network of hospitals, SNFs, physicians, and senior communities.

  • Build and maintain a robust Albuquerque referral network spanning hospitals, SNFs, physicians, and senior networks.
  • Educate referrers on hospice eligibility, services, and care coordination while coordinating handoffs with intake and clinical leadership.
  • Increase community presence through in-person outreach, events, and meetings to drive referrals and census.

Job Description

Responsibilities

  • Develop and execute strategic marketing plans to grow Del Corazon Hospice census in Albuquerque
  • Build and maintain referral relationships with: Hospitals and discharge planning teams, Skilled nursing and long-term care facilities, Physicians, specialists, and clinics, Assisted living communities and senior networks
  • Conduct regular in-person outreach, education, and follow-up with referral sources
  • Serve as a hospice resource by providing education on eligibility, services, and care coordination
  • Collaborate closely with intake and clinical leadership to ensure smooth referral transitions
  • Attend community events, facility meetings, and networking opportunities
  • Track outreach activity, referral trends, and performance outcomes
  • Represent Del Corazon Hospice with professionalism, compassion, and urgency

Requirements

  • Prior experience in hospice marketing, home health marketing, or healthcare business development preferred
  • Established referral relationships in the Albuquerque healthcare community strongly valued
  • Knowledge of hospice services, Medicare guidelines, and admissions processes
  • Strong communication, organizational, and interpersonal skills
  • Self-driven with the ability to work independently in the field
  • Valid driver’s license and reliable transportation required

  • Established referral relationships in the Albuquerque healthcare community strongly valued
