Job Summary
Own the admissions and census strategy in a SNF, aligning with admins to hit census targets, manage referrals, and ensure PDPM compliant documentation and HIPAA readiness.

Responsibilities
- Own relationship-building with physicians, case managers, social workers, and discharge planners to drive timely referrals.
- Ensure every new admission has proper discharge summaries, therapy notes, nursing notes, and medication lists to stay PDPM compliant.
- Coordinate with Admissions and marketing to promote the facility and maintain a robust community presence.
- 1 year of SNF marketing/admissions experience
- 1 - 5 years of marketing/admissions experience in healthcare
- Fully vaccinated against COVID-19 including booster shots
- Previous experience in marketing or other related fields
- Strong leadership qualities
