Job Description

Responsibilities

  • Develop and maintain strong relationships with physicians, contracted hospitals & healthplans, assisted living communities, case managers, discharge planners, and other healthcare professionals to generate hospice referrals.
  • Educate patients, families, caregivers, and referral sources about hospice eligibility, services, and the benefits of hospice care.
  • Conduct hospice benefit presentations and in-service education for hospitals, healthcare facilities, contracted health plans, and community organizations.
  • Meet with patients and families to provide compassionate hospice benefit explanations (EOBs) and answer questions regarding services.
  • Serve as a trusted resource for referral partners by providing timely follow-up and exceptional customer service.
  • Collaborate closely with the clinical team to ensure seamless patient admissions and continuity of care.

Requirements

  • Bachelor's degree preferred.
  • Minimum 3 years of recent hospice marketing or hospice liaison experience (required).
  • Demonstrate success developing referral relationships within the healthcare community.
  • Excellent communication, presentation, and interpersonal skills.
  • Compassionate, professional, and patient-centered approach.
  • Self-motivated with the ability to work independently while collaborating effectively with the team.
  • Valid California driver's license, reliable transportation, and willingness to travel throughout the assigned territory.
  • Willingness to be available for patients, family members, and / or referring parties outside of normal business hours
  • Strong organizational, time management, and marketing skills.
