Track activity and follow up on referral opportunities with weekly reporting.

Job Description

Responsibilities

Develop and maintain referral relationships throughout the Dallas area
Conduct in-person visits to hospitals, physician offices, rehabilitation facilities, assisted living communities, nursing facilities, and community organizations
Educate referral sources about agency services
Attend networking events, health fairs, and community outreach activities
Follow up with referral sources and referral opportunities
Submit weekly activity reports
