Responsibilities

  • Lead census development and hospital referral strategy across the Jacksonville market
  • Build and maintain relationships with hospital case managers, discharge planners, and physicians
  • Strengthen and expand referral networks across multiple facilities
  • Partner with admissions teams to improve conversion and streamline referrals
  • Identify opportunities to grow market presence and increase admissions volume

Requirements

  • 5+ years in healthcare marketing, admissions, or business development
  • Skilled Nursing or post-acute experience strongly preferred
  • Proven success working directly with hospital case managers and discharge planners
  • Experience managing relationships across multiple facilities or a defined territory preferred
  • Strong communication, leadership, and relationship-building skills
