Job Summary
Own and grow a defined territory by building trusted referral networks, representing the agency in community settings, educating partners and families about Medicaid/Medicare services, and tracking outcomes in the CRM.
- Territory ownership: grow referrals by engaging case managers, social workers, community organizations, and other Medicaid sources.
- Community presence: attend events, health fairs, and outreach programs to stay a trusted local resource.
- Education and navigation: explain home care options under Medicaid, Medicare GUIDE, and VA, and how Home or Away supports clients and caregivers.
