Job Description

Responsibilities

  • Create and post outreach content on Instagram, Facebook, LinkedIn, TikTok, and other approved platforms.
  • Help promote ThriveWorks services to community members seeking support with healthcare access, housing, food, transportation, benefits, and other health-related social needs.
  • Research and help maintain ThriveWorks listings on resource platforms such as 211 LA, One Degree, Google Business Profile, Find211.org, and other community-resource directories.
  • Assist with outreach to healthcare providers, community organizations, schools, workforce partners, and referral sources.
  • Use Jotform to collect prospective member information, referrals, and outreach responses using approved forms.
  • Enter and maintain member engagement information in Microsoft Excel, including outreach contacts, referral activity, follow-up status, and connection outcomes.
  • Keep outreach and member-engagement records organized, accurate, and up to date.
  • Help track inquiries, referrals, follow-ups, and outreach results.
  • Support community events, resource fairs, and digital campaigns.
  • Follow ThriveWorks confidentiality, privacy, and professional communication requirements.

Requirements

  • Comfortable using social media for marketing and outreach.
  • Familiar with or willing to learn Canva, Excel, Jotform, Google Workspace, and Microsoft Office.
  • Strong written and verbal communication skills.
  • Comfortable researching community resources online.
  • Organized and able to manage data entry and follow-up tasks.
  • Interested in community health, marketing, healthcare navigation, social services, or digital communications.
  • Prior outreach, marketing, or CHW experience is helpful but not required.
