Position Summary:
Gracious Care Hospice is seeking an experienced Community Liaison with an established presence in the Portland Metro healthcare community. This role is ideal for a clinically knowledgeable professional who excels in building trusted relationships and serving as a resource to providers, patients, and families navigating hospice care.

The Community Liaison will collaborate closely with hospitals, adult foster homes, assisted living facilities, memory care communities, and skilled nursing facilities to support appropriate hospice referrals and ensure continuity of care.

Key Responsibilities:

• Develop and maintain strong, ongoing relationships with hospitals, adult foster homes (AFHs), assisted living, memory care, and skilled nursing facilities (SNFs) across Multnomah, Washington, and Clackamas counties
• Serve as a clinical resource by educating providers, staff, patients, and families on hospice philosophy, eligibility, and services
• Facilitate timely and appropriate hospice referrals through proactive communication and collaboration with interdisciplinary teams
• Support smooth transitions of care from acute or residential settings to hospice services
• Conduct routine facility visits to maintain visibility and strengthen partnerships
• Communicate effectively with hospice clinical team to ensure alignment on patient needs and admissions
• Participate in care coordination discussions as appropriate with facility staff
• Maintain accurate records of referral activity and interactions

Qualifications:

• Minimum of 3 years of experience in hospice, home health, or healthcare liaison role
• Demonstrated, established relationships within local hospitals and long-term care communities required
• Strong working knowledge of hospice eligibility criteria and clinical processes
• Experience working with AFHs, assisted living, memory care, and SNFs strongly preferred
• Excellent interpersonal, communication, and relationship-building skills
• Ability to engage clinical staff and leadership in meaningful, trust-based partnerships
• Highly organized and able to work independently in a field-based role
• Valid driver’s license and reliable transportation

Education:

• Bachelor’s degree in healthcare, nursing, marketing, or related field preferred (or equivalent experience)

Work Environment:
This is a field-based position requiring regular travel throughout the Portland Metro area, including Multnomah, Washington, and Clackamas counties.

Pay: $65,000.00 - $100,000.00 per year
