Job Summary

Own patient referrals and hospital-to-care transitions by coordinating referrals, admissions, and bedside engagement with referral sources.

  • Referral Management: Review clinical appropriateness and respond promptly to referrals.
  • Relationship & Coordination: Build and maintain referral-source relationships to ensure smooth transitions.
  • Operational Ownership: Align with the Admission Director on eligibility and bedside presence to accelerate placement.

Job Description

Responsibilities

  • Manage all referrals, reviewing for clinical appropriateness
  • Ensure all referrals are being responded to promptly
  • Work with the Admission Director to establish patient eligibility
  • Provide bedside sales and frequent hospital rounding
  • Coordinate with the facility to ensure a smooth transition from the hospital
  • Build strong relationships with referral sources, and maintain these relationships

Requirements

  • 1 year of experience in a skilled nursing setting
  • Strong clinical and marketing skills
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
