Job Summary

Own patient referrals and hospital-to-care transitions by coordinating referrals, admissions, and bedside engagement with referral sources.

Referral Management: Review clinical appropriateness and respond promptly to referrals.
Relationship & Coordination: Build and maintain referral-source relationships to ensure smooth transitions.
Operational Ownership: Align with the Admission Director on eligibility and bedside presence to accelerate placement.

Job Description

Responsibilities

Manage all referrals, reviewing for clinical appropriateness
Ensure all referrals are being responded to promptly
Work with the Admission Director to establish patient eligibility
Provide bedside sales and frequent hospital rounding
Coordinate with the facility to ensure a smooth transition from the hospital
Build strong relationships with referral sources, and maintain these relationships

Requirements

1 year of experience in a skilled nursing setting
Strong clinical and marketing skills
Excellent communication and interpersonal skills
Ability to work in a fast-paced environment and prioritize tasks effectively
