Job Summary

Own the admissions funnel from referral generation to placement, with a focus on cross functional coordination and hitting census and revenue goals.

  • Referral Relationships: Build and maintain relationships with referral sources to generate admissions.
  • Admissions & Process: Coordinate payer source identification through patient analysis to streamline the flow.
  • Cross-functional Collaboration: Align with physicians, nursing, rehab, and social work for seamless transfers and accurate clinical information.

Job Description

Responsibilities

  • Establish and maintain positive relationships with referral sources to promote skilled services
  • Coordinate the referral process from the identification of the appropriate payer source to the analysis of patient information
  • Responsible for achieving the proposed company census and revenue objectives
  • Collaborate with patient or representative, physicians, residents, nursing staff members, rehabilitation therapy staff members, and social work staff members to ensure seamless transfer to the facility
  • Communicates information effectively to physicians, supervisors, facility administrators, and any other pertinent personnel in a timely manner
  • Analyze patient needs and match with facility services to plan for an appropriate facility placement
  • Complete an on-site visit to the referring facility to review the physician orders, assess the patient’s clinical needs, and obtain clinical information
  • Other duties as assigned by supervisor

Requirements

  • Prior sales and marketing experience in the healthcare industry preferred
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) qualifications optional
  • Minimum of two (2) year experience in long-term care
  • Proven ability to successfully market the services of the organization.
