Job Summary

Own growth and partnerships strategy to drive referrals and admissions while delivering a seamless, patient-centered experience in post-acute care.

  • Growth & Partnerships: Build hospital, case manager, physician, discharge planner, ACO, and community relationships to expand referrals.
  • Operations & Experience: Manage admissions pipeline, ensure timely communication, and coordinate transitions of care to optimize the patient journey.
  • Market & Representation: Serve as the local face, engage in events, and leverage CRM data to drive measurable growth.

Job Description

Own growth and partnerships strategy to increase referrals and admissions while ensuring a seamless, patient-focused experience in post-acute care.

  • Growth & Partnerships: Build hospital, case manager, physician, discharge planner, ACO, and community partnerships to expand referrals and admissions.
  • Operations & Experience: Oversee the admissions pipeline, ensure timely communication, guide transitions of care, and collaborate to deliver exceptional customer experience.
  • Market & Representation: Serve as the community face, represent the organization at events, and use CRM/admissions data to drive measurable growth.

  • Experience in healthcare marketing, admissions, business development, or census development.
  • A proven ability to build trust, influence decision-makers, and grow referral networks.
  • Strong communication, presentation, and relationship-management skills.
  • A data-driven mindset with the ability to turn insights into action.
  • Entrepreneurial energy, initiative, and a passion for achieving results.
  • Experience with CRM systems and Microsoft Office applications.
  • Skilled nursing, rehabilitation, long-term care, post-acute care, or healthcare sales experience preferred.
