Job Summary

Own referrals-to-admissions pipeline and community partnerships to grow census in a skilled nursing setting.

  • Own referral development and admissions coordination across hospitals, clinics, and community sources to drive steady census growth and timely move-ins.
  • Lead tours, follow-ups, and relationship-building with families and referral partners to ensure a welcoming admission experience and smooth intake.

Job Description

Responsibilities

  • Promote Emerald Nursing and Rehab Columbus through community outreach, marketing efforts, and relationship building.
  • Develop and maintain strong relationships with hospitals, clinics, discharge planners, case managers, social workers, community organizations, and other referral sources.
  • Serve as a positive and professional representative of Emerald Nursing and Rehab in the community.
  • Attend community events, networking opportunities, referral meetings, and outreach activities as needed.
  • Follow up on referral opportunities and maintain consistent communication with referral partners.
  • Track referrals, admissions, census trends, and follow-up opportunities.
  • Provide facility tours to potential residents, families, referral partners, and community members.
  • Create a positive, welcoming, and supportive experience for potential residents and families.
  • Assist with developing marketing materials, social media content, and facility promotional efforts.
  • Support census growth through consistent outreach, follow-up, and relationship development.

Preferred

  • Bachelor’s degree in healthcare administration, social work, marketing, business, psychology, or a related field preferred.
  • Equivalent combination of education and experience may be considered.
  • Prior experience in long-term care admissions, skilled nursing admissions, healthcare marketing, sales, community relations, or referral development preferred.
  • Knowledge of Medicare, Medicaid, Managed Care, HMOs, insurance verification, and reimbursement processes preferred.
  • Experience with PointClickCare or other electronic health record systems preferred.
