Job Description

Responsibilities

  • Direct social work services to nursing home and rehabilitation patients and families as needed.
  • Provide administrative and clinical supervision to social workers and discharge planners to ensure quality of service.
  • Ensure department documentation complies with state and federal regulations.
  • Plan, develop, organize, implement, evaluate and direct the operations of social service programs.
  • Develop policies and procedures for identifying the medically-related social and emotional needs of patients/residents.
  • Assist in solving social and practical environmental problems, including discharge planning and referrals to community agencies.
  • Provide supervision to staff to meet regulatory compliance.
  • Provide clinical consultation, oversight of case planning and care coordination.
  • Direct crisis intervention efforts and intervene as required.
  • Provide staff management including hiring, development, training, and performance management.

Requirements

  • Licensed SW in the State of Maryland required
  • Degree in Social Work required

Preferred

  • Minimum of 1 year Medical Facility experience preferred (Nursing Home or Hospital)
  • Experience in Adult or Geriatric population preferred
