Responsibilities

• Build and nurture strong, lasting relationships with: Hospitals, Skilled Nursing Facilities (SNFs), Assisted Living & Memory Care communities, Adult Foster Homes (AFHs)
• Serve as a trusted resource to providers, discharge planners, and care teams
• Educate partners on: Hospice eligibility and timing, Clinical appropriateness, The value of early hospice involvement
• Support patients and families by helping ensure timely, smooth transitions to hospice care
• Maintain a consistent presence in the community through in-person visits and follow-up
• Grow and manage a strong referral network across the region
• Track referral activity and collaborate with the clinical team to support admissions
• Contribute to thoughtful, sustainable growth aligned with our mission and values

Requirements

• Minimum 4 years of direct hospice marketing/outreach experience REQUIRED
• Established referral relationships in the Portland Metro area REQUIRED
• Proven track record of generating hospice referrals
• Strong understanding of hospice eligibility, regulations, and care processes
• Excellent communication and relationship-building skills
• Self-motivated, organized, and comfortable working independently in the field
• Valid driver’s license and reliable transportation
