Responsibilities

• drive growth through strategic partnerships, community outreach, and referral development
• build strong relationships with hospitals, skilled nursing facilities, rehabilitation centers, physician offices, clinics, case managers, social workers, and other healthcare professionals while connecting eligible patients with quality home health services

Requirements

• Home health or healthcare marketing/business development experience preferred
• Established healthcare referral relationships are a plus
• Strong communication, networking, and relationship-building skills
• Ability to generate and develop qualified home health referrals
• Knowledge of Medicare home health and community referral sources preferred
• Self-motivated, professional, organized, and results-oriented
• Passion for helping patients safely receive quality care at home
