Responsibilities

• Visit hospitals, rehabilitation centers, skilled-nursing facilities, hospice agencies, home-health agencies, medical offices, senior centers, and placement agencies.
• Build professional relationships with discharge planners, case managers, social workers, placement coordinators, and healthcare providers.
• Distribute brochures and provide information about our services and available rooms.
• Follow up with prospective residents, families, and referral contacts.
• Help arrange facility tours.
• Maintain accurate records of visits, contacts, referrals, mileage, and follow-up activities.
• Submit an activity report at the end of each working day. Represent Evergreen Carehome Inc. professionally and protect resident confidentiality.

Requirements

• Experience in healthcare, senior living, RCFE, hospice, home health, sales, marketing, or community outreach is preferred.
• Knowledge of the Fresno and Clovis healthcare community is a plus.
• Existing relationships with local healthcare providers are highly desirable.
• Must have strong communication and relationship-building skills.
• Must be professional, dependable, and able to work independently.
• Basic smartphone and computer skills are required.
