Educates and coordinates with case managers, social workers, and clinicians to ensure smooth transitions and ongoing engagement

Job Description

Responsibilities

Increase census through cultivating relationships with multiple referral sources including hospitals, LTACs, ALs, ILs, and the community at large
Assess patients and work to build relationships with area hospital case managers, social workers and physicians
Attend hospital meetings, coalition meetings, and networking meetings to grow brand awareness
Reflect the quality of care and services provided, including on-site short-term, respite, and long-term care in our skilled nursing facility
Act as a self-starter, skilled communicator and positive motivator
Able to organize and prioritize many tasks effectively
Ability to utilize both local and company resources in the execution of job responsibilities
Must have the ability to network at all levels with strong drive, a passion for success, and a proven track record of census growth and development
