Job Summary

Own referral development and partner education to expand access to Hope programs, partnering with clinicians and providers to drive enrollments and appropriate admissions.

  • Referral Development: Expand sources and convert referrals across Hospice, PACE, and related Hope programs.
  • Provider Education: Educate and enable providers to refer and enroll patients effectively.
  • Clinical Alignment: Collaborate with clinical leadership to meet access goals and improve patient outcomes.

Job Description

Own community-access expansion for Hope programs by expanding referrals, educating providers, and aligning clinical partners to enroll patients.

  • Referral Development: Identify and increase referral sources to meet organizational access-to-care goals.
  • Community Education: Provide education to communities when requested to support program enrollment.
  • Provider Relationship Building: Identify opportunities with other health care providers to support enrollment into assigned Hope programs.
  • Referral Source Conversion: Ensure referral sources choose a Hope Healthcare program for their patients or residents.
  • Sales Calls & In-Services: Educate referral sources about program services using planned calls and literature.
  • CRM & Documentation: Document daily sales calls and maintain referral-partner information in the CRM.
  • Market Awareness: Communicate market environment and opportunities to clinical leadership to inform strategy.
  • Territory Coverage: Travel within Hope Healthcare service areas to meet access goals.
  • Collaboration with Clinical Leadership: Work with clinical leaders to align referral-partner needs and patient expectations.
  • Program Knowledge: Articulate features, benefits, and Medicare/Medicaid guidelines of assigned Hope programs.

Requirements

  • Minimum of 2 years of proven healthcare sales performance with a degree in marketing, business, public relations or related field or background in healthcare such as nursing, social work, or other patient centered healthcare role.
  • Must possess a valid Florida Drivers' License, current auto insurance, and reliable transportation.
  • R Ns, Social Workers must keep all licensing current.
  • Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
  • Ability to calculate figures and amounts such as averages, proportions, and percentages. Must be able to differentiate mean, mode and average and understand basic concepts of regression analysis.
  • Ability to listen intently and articulate Hope’s position in relation to objections. Ability to communicate, concentrate and document effectively and accurately.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Able to travel within the entire Hope Healthcare service areas and assigned territory
  • Achieve monthly and quarterly access goals for assigned program.
  • Successfully execute a weekly, monthly and quarterly strategy to increase market share.
