Build and sustain a robust referral network in Erie, aligning partner relationships with growth targets and smooth client onboarding.

Partnership Development: Own referral-source relationships and drive ongoing partner engagement.
Onboarding & Collaboration: Coordinate cross-team onboarding to activate new clients quickly.
Market Insight & Expansion: Track trends and identify opportunities to expand AHHS’s footprint.

Job Description

Partnership Development: Build and maintain relationships with referral sources including Managed Care Organizations, hospitals, physician offices, rehabilitation centers, and community organizations.
Market Expansion: Identify new business opportunities and expand AHHS’s footprint in the Erie market.
Partner Engagement: Conduct regular in-person visits, presentations, and follow-ups with referral partners.
Education & Advocacy: Educate potential clients and partners about AHHS services and value.
Program Coordination: Work with PA's Home and Community Based Waiver program.
Onboarding & Collaboration: Collaborate with internal teams to ensure smooth onboarding of new clients.
Performance Tracking: Track referral activity, maintain accurate records, and meet growth targets.
Community Representation: Represent AHHS at networking events, health fairs, and community outreach initiatives.
Industry Awareness: Stay informed on industry trends and competitor activity.

Previous experience in home care marketing is required.
Strong communication, networking, and relationship-building skills.
Self-motivated with the ability to work independently and meet goals.
Reliable transportation and willingness to travel throughout Erie and surrounding counties.
Proficiency with CRM systems and Microsoft Office.

Knowledge of the local healthcare landscape is a plus.
