Responsibilities

• Develop and maintain strong relationships with referral sources including Managed Care Organizations, hospitals, physician offices, rehabilitation centers, and community organizations
• Work with PA's Home and Community Based Waiver program
• Conduct regular in-person visits, presentations, and follow-ups with referral partners
• Identify new business opportunities and expand AHHS’s footprint in the Erie market
• Educate potential clients and partners about AHHS services and value
• Track referral activity, maintain accurate records, and meet growth targets
• Collaborate with internal teams to ensure smooth onboarding of new clients
• Represent AHHS at networking events, health fairs, and community outreach initiatives
• Stay informed on industry trends and competitor activity

Requirements

• Previous experience in home care marketing is a MUST HAVE
• Strong communication, networking, and relationship-building skills
• Self-motivated with the ability to work independently and meet goals
• Reliable transportation and willingness to travel throughout the Erie and the surrounding counties
• Knowledge of the local healthcare landscape is a plus
• Proficiency with CRM systems and Microsoft Office
