Job Summary

Own and grow the Nashville hospital referral program by building strong hospital and case-manager partnerships, driving referrals into Eaton Creek, and achieving census growth through coordinated marketing and admissions efforts.

  • Track referrals, conversions, and census metrics to identify improvements and drive occupancy.
  • Collaborate with Administration and Admissions to convert referrals into admissions and maintain partner communication.

Job Description

Responsibilities

  • Develop and maintain established relationships with Nashville-area hospitals and case managers to generate consistent referral opportunities.
  • Serve as a primary liaison between Eaton Creek, hospitals, discharge planners, and other referral sources.
  • Proactively market Eaton Creek’s clinical capabilities, services, and strengths to referral partners.
  • Work closely with the Administrator and Admissions Department to convert referrals into admissions and maximize occupancy.
  • Track referral activity, conversion rates, and census growth while identifying opportunities for improvement.
  • Maintain strong communication and follow-up with referral sources throughout the admissions process.
  • Represent Eaton Creek professionally and build a strong reputation within the Nashville healthcare community.
  • Ideal Candidate - Established relationships with Nashville-area hospitals and case managers required. - Demonstrated track record of increasing SNF census and generating referrals. - Strong understanding of the hospital-to-SNF discharge and referral process. - Excellent communication, networking, and relationship-building skills. - Highly motivated, results-oriented, and comfortable working in a fast-paced admissions environment. - Previous SNF, healthcare marketing, hospital liaison, or business development experience preferred.

Requirements

  • Must possess, as a minimum, A Bachelor’s Degree from an accredited college/university, or equivalent. (Five (5) years experience in marketing/public relations in a health care setting may be recognized in lieu of a Bachelor’s Degree.)
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from managers and employees.
  • Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Certificates, Licenses, Registrations: To perform this job successfully, an individual must be proficient in the Microsoft Suite products.
