Job Summary

Own and drive Hemet Hills Post Acute’s community referral development and census growth by building hospital and partner relationships, coordinating admissions with the clinical team, and tracking marketing metrics.

Job Description

Responsibilities

  • Develop and maintain strong relationships with referral sources throughout the community
  • Conduct regular visits to hospitals, physician offices, assisted living communities, and other healthcare partners
  • Promote facility services, clinical programs, and specialty offerings
  • Coordinate referral admissions with the Admissions and Clinical teams
  • Monitor referral activity, admissions trends, and census development opportunities
  • Participate in community outreach, networking events, and healthcare industry activities
  • Collaborate with facility leadership to develop strategic marketing plans
  • Track and report marketing efforts, referrals, and conversion metrics
  • Maintain knowledge of healthcare regulations and compliance requirements related to marketing activities
  • Serve as a positive ambassador for Hemet Hills Post Acute and PACS

Requirements

  • Previous healthcare marketing, business development, admissions, or sales experience preferred
  • Skilled Nursing Facility (SNF), post-acute, home health, hospice, or healthcare experience strongly preferred
  • Excellent communication and relationship-building skills
  • Proven ability to establish and maintain professional partnerships
  • Strong organizational and time management abilities
  • Self-motivated with a track record of achieving goals and outcomes
  • Proficiency with Microsoft Office and CRM software preferred
  • Valid driver's license and reliable transportation required
