Job Summary

Own and grow community partnerships and outreach to connect patients with hospice care, while coordinating with the clinical team to ensure smooth transitions and location growth.

  • Community & Provider Partnerships: Build and expand referral networks with providers, facilities, and community partners.
  • Education & Outreach: Lead in-services and events to raise awareness of hospice services for families and caregivers.
  • Care Coordination Support: Align outreach with clinical goals to support timely transitions and location growth through proactive leadership.

Job Description

Responsibilities

  • Develop and grow referral partnerships throughout the community.
  • Serve as the face of Concho Hearts Hospice in your service area.
  • Visit providers, facilities, and community partners to share our mission.
  • Lead educational in-services and outreach events.
  • Help families understand hospice services and expectations.
  • Collaborate closely with the clinical team to ensure smooth care transitions.
  • Support the growth and success of your location through strong leadership.

Requirements

  • Minimum of 2 years of hospice experience required (marketing, leadership, or operations).

Preferred

  • Hospice and/or home health marketing experience preferred.
  • Strong leadership and relationship-building skills.
  • Excellent communication and organizational abilities.
  • A heart for serving patients and families.
