Job Description

Develop and maintain positive relationships with hospitals, physicians offices, nursing homes, assisted and supportive living facilities, faith based organizations and community groups.
Establishing and maintaining strong relationships with hospital social workers, case managers, to facilitate referrals.
Establishing and maintaining strong relationships with the nursing homes administrators and facilitate contract establishments.
Provide accurate information to patients, families, referral sources regarding hospice, eligibility, and benefits.
Organize and patriciate in community events, health fairs, and educational seminars to promote hospice services.
Act as an advocate for patients and families throughout the transition to hospice care.
Maintain accurate records of referrals, admissions, and follow-up communications.
Stay informed about changes in healthcare regulations and hospice best practices.
