Responsibilities

• Home Health Marketer develops referral relationships throughout Riverside and San Bernardino Counties to increase awareness of Care Essentials home health services. This position educates referral sources, strengthens community partnerships, and helps ensure timely, appropriate patient transitions into home health services.

• Develop relationships with:
• Hospitals
• Physicians
• Skilled Nursing Facilities
• Assisted Living Communities
• Case Managers
• Discharge Planners
• Wound Clinics

Requirements

• 2+ years home health marketing
• Medicare home health experience
• Existing Inland Empire relationships preferred
• Reliable transportation
• Excellent communication skills
