Job Summary
Own the local outreach and intake flow by building a referral network, converting leads into clients, and coordinating care with scheduling and care teams.
- Referral network: expand partnerships with hospitals, rehab centers, and community providers and maintain weekly outreach.
- Client onboarding: consult families, explain services and funding options, and drive intake coordination.
- Care coordination and operations: support caregiver recruitment, scheduling, and office processes to ensure smooth client experiences.
Job Description
