Job Summary
Own the local referral network and census growth by identifying sources, building relationships, and guiding enrollment for seniors and disabled populations.
- Identify and prioritize referral sources such as independent living facilities, assisted living, adult homes, shelters, and senior housing
- Facilitate enrollment and enrollment orders, doctor orders, and appointments for members
- Present to consumer and professional groups to expand partnerships and drive referrals
Job Description
Responsibilities
- Identifying, evaluating, and prioritizing potential referral sources within the agency’s territory and surrounding area
- Establishing and maintaining professional relationships with all referral sources, including but not limited to the following: independent living facilities, assisted living facilities, Adult Home, Shelters, & Senior Housing.
- Developing a network of referral sources.
- Obtaining Enrollment information
- Assisting members in obtaining doctor's orders
- Assisting Members with Enrolment Appointments
- Identifying and marketing to seniors and disabled persons and to Buildings that serve such populations;
- Conducting presentations to consumers and professional groups;
- Marketing to Medicaid and Medicare beneficiaries;
- Developing and maintaining new referral sources and/or partnerships in the community.
