Responsibilities

• This role visits local hospitals and skilled nursing facilities to let them know about Care Planning Institute’s quality home care aide services.
• This role also talks to potential clients about CPI services and often visits with them in the client’s home to build a relationship.
• This role hands clients off to one of CPI Case Managers to manage the client onboarding and ongoing services.

Requirements

• MUST have experience working with seniors in Home Care or Assisted Living.
• Applicant MUST have existing relationships they can evidence with social workers, discharge planners, etc. at local hospitals and SNFs.
• Passing our screening/background check is also required for employment.
