Job Summary
Own or manage a community-facing territory by building trusted referral partnerships with case managers, social workers, and community groups, educating families on Medicaid/home care options, and tracking progress in the CRM to drive measurable referrals.
- Field & Partnerships: Build and maintain community partnerships, represent the company at events, and convert outreach into referrals.
- Education & Support: Educate partners and families on Medicaid-based home care and caregiver options.
- Measurement & Accountability: Track activities and referrals to ensure results and growth.
Job Description
Own the defined territory and drive referrals through trusted community partnerships and hands-on outreach.
- Growth and Partnerships: Own a defined territory and grow referrals significantly.
- Community Visitation: Spend your days out in the community visiting Case managers, Social workers, Community organizations, Other Medicaid referral sources.
- Presence & Representation: Be a consistent, trusted presence in the communities we serve; attend and represent Home or Away at Community events, Health fairs, Outreach programs.
- Partner Education: Educate referral partners and families on Home care services offered through Medicaid, Medicare GUIDE, and Veteran Affairs.
- Caregiver Support: Educate on how Home or Away supports clients and caregivers, including family caregivers.
- CRM & Accountability: Track your outreach activity and referrals in our CRM; be accountable for results, not just activity.
