Job Summary

Drive referral growth and relationship-building for a Home Health Community Liaison in Katy/West Houston.

  • Develop and maintain referral relationships with physicians, hospitals, SNFs, and rehab centers.
  • Own territory growth by driving referral volume in Katy and West Houston.
  • Align intake, clinical, and operations to ensure timely admissions and conversions.

Job Description

Responsibilities

  • Develop and maintain strong referral relationships with physicians, hospital case managers, discharge planners, skilled nursing facilities, rehab centers, and other healthcare partners
  • Drive consistent referral growth in the Katy and West Houston service area
  • Conduct daily in-person outreach, facility visits, and relationship-building activities
  • Educate referral sources on agency services, admission criteria, payer mix, and points of differentiation
  • Coordinate closely with intake, clinical, and operational teams to ensure smooth referral conversion and timely admissions
  • Track referral activity, conversion trends, and territory performance
  • Identify new business opportunities and maintain a strategic plan for market penetration
  • Represent the agency professionally at networking events, community functions, and healthcare meetings
  • Maintain compliance with all applicable federal, state, and agency marketing guidelines

Requirements

  • Minimum of 2 years of successful experience in home health marketing, liaison, account executive, or business development roles
  • Demonstrated success building referral volume in the home health space
  • Established knowledge of the Katy, West Houston, and surrounding referral market
  • Strong working relationships with physicians, hospitals, SNFs, and discharge planning teams preferred
  • Excellent communication, presentation, and relationship-management skills
  • Ability to work independently and manage a defined territory with accountability for results
  • Professional appearance, strong organization, and reliable follow-through
  • Valid driver’s license and dependable transportation

Preferred

  • Existing referral relationships in the Katy market
  • Prior success launching or rapidly growing census for a home health agency
  • Experience working with Medicare-certified home health agencies
  • Familiarity with EMR/intake processes, referral coordination, and payer considerations
  • Background in healthcare sales, territory development, or community outreach
