Overview

Valerian Home Health and Hospice is seeking a compassionate, motivated, and relationship-driven Home Health & Hospice Liaison to join our growing team. This position is responsible for promoting the agency's hospice and home health services, developing referral relationships, generating new business opportunities, increasing referrals from existing accounts, and identifying community needs. This role combines relationship building, strategic marketing, community outreach, and business development to support agency growth while maintaining a patient-centered focus.

Responsibilities

• Build, develop, and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, senior living communities, case managers, discharge planners, and other referral sources.
• Educate patients, families, and healthcare professionals to increase awareness, strengthen community presence, and drive patient referrals.
• Conduct regular in-person visits with referral sources to build trust, gather feedback, ensure satisfaction, and identify opportunities for additional services.
• Collaborate with clinical leaders, liaisons, management, administrative staff, and leadership teams to ensure a smooth admission process.
• Identify, evaluate, and pursue new leads based on market potential, geographic opportunities, and community needs.
• Represent Valerian Home Health and Hospice professionally throughout the community.

Qualifications

• Previous experience in healthcare, home health, hospice, senior living, physician relations, case management, discharge planning, business development, marketing sales, or a related healthcare field.
• Strong relationship-building and customer service abilities with excellent communication skills.
• Professional, organized, and goal driven.
• Strong computer literacy and proficiency with Microsoft Office applications.
• Working knowledge of HIPAA regulations and healthcare compliance requirements.
• Reliable transportation required.

Preferred Qualifications

• Bachelor's or Nursing degree preferred (Healthcare Administration, Marketing, Communications, Business, or related field).
• Established referral source relationships within the healthcare community.
• Proven track record of meeting or exceeding referral growth.
• Home Health or Hospice experience, a plus.
