Job Summary
Own community-facing hospice liaison efforts to build referrals, grow census, and ensure seamless admissions by aligning clinical teams, educating local providers, and representing the organization with compassion.
- Community Engagement: Develop relationships with hospitals, physicians, and referral sources to drive patient admissions.
- Local Marketing & Outreach: Promote hospice services in Lake Havasu through events, education, and community activities.
- Collaboration & Care Continuity: Coordinate with clinical leadership to support admissions and continuity of care, and maintain referral activity records.
Job Description
- Develop and maintain relationships with hospitals, physicians, nursing facilities, assisted living communities, and other referral sources
- Promote hospice services within the Lake Havasu community through outreach and education
- Identify opportunities to grow census and strengthen community presence
- Coordinate and participate in marketing events, meetings, and community activities
- Collaborate closely with clinical and leadership teams to support patient admissions and continuity of care
- Maintain accurate records of referral activities and community interactions
- Assist in developing marketing strategies and outreach initiatives
- Represent the organization professionally and compassionately at all times
- Support organizational goals through effective communication and relationship management
