Responsibilities

• Understands and implements the principles of community integration. Assisting individuals with seeking, attaining and engaging in beneficial activities in the community. Uses appropriate assessment tools to gain a deeper understanding of each individual’s desires, strengths, and challenges.
• Maintains positive relations and builds rapport with members of the community to ensure meaningful relationships are created .
• Maintain collaborative relationships with non-profit organizations (501(c)3) to create and retain future volunteer opportunities.
• Through community resources, individuals are able to engage in community activities, by way of local businesses, and governmental entities to provide enriching opportunities that foster their highest potential outcome.
• Finds and provides community resources to support the individuals we serve.
• Track and document progress daily for billing purposes.
• Collaborates with team members to identify community resources and volunteer opportunities.
• Maintains, creates, and submits a weekly person-centered schedule reflecting and ensuring meaningful activities in the community.
• Completes person centered annual assessments and attends IDT meetings as necessary.
• Maintains and updates individuals’ file(s) to ensure the most current information is available.

Requirements

• EDUCATION and/or EXPERIENCE: High School diploma or GED. Two or more years of experience in serving individuals with IDD, preferred. Must be proficient in Microsoft Suite (Word, Outlook, Excel).
• LANGUAGE SKILLS : Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
• MATHEMATICAL SKILLS : Ability to add, subtract, multiply, and divide.
• REASONING ABILITY : Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several variables.
• CERTIFICATES, LICENSES, REGISTRATIONS : Must have reliable transportation. Must possess and maintain a valid California Driver's License, auto insurance, car registration. Standard First Aid and CPR certification preferred. Certification will be provided within the first 90 days of employment at the expense of the Agency.
