Job Summary

Coordinate ED to primary care handoffs and connect patients with PCPs and community resources while removing access barriers, boosting preventive and chronic care adherence, and driving down avoidable ED visits across multiple sites.

  • Care Coordination: Link ED patients to PCPs and community resources at scale.
  • Access & Adherence: Remove barriers, improve preventive and chronic care, and support self-management.
  • Outcome & Data: Collect and report data to shrink preventable ED visits and hospital admissions.

Job Description

  • Support and facilitate relationships and communication with patients seeking emergency department care for primary care services and link them with a primary care physician and community resources.
  • Address barriers to accessing primary care to receive appropriate care, improve preventive and chronic care compliance, while supporting patient self management for underlying health issues.
  • Improve health outcomes and reduce preventable emergency department visits and potential hospital admissions.
  • Demonstrate commitment to working within a team of health professionals, accepting and considering all points of view
  • Demonstrate ability to collect and report data accurately
  • Demonstrate Catholic health values and expected behaviors in daily interactions
  • Ability to drive and travel to multiple sites
  • Master of Social Work (MSW) from an CSWE-accredited program or
  • Bachelor of Social Work with a Master’s Degree in Healthcare or related field
  • Current NYS LMSW or LCSW license preferred
  • Two years of healthcare/community health/social services/human services preferred
  • Five years of social work experience required if BSW
  • Two or more years of experience in telephonic support and electronic medical record experience preferred
  • Prior telephonic support and electronic medical record experience preferred
