Are you a nurse or clinically experienced healthcare professional ready to make a meaningful impact beyond bedside care? We’re looking for a compassionate, relationship-driven Clinical Liaison to join the Evergreen Healthcare Group team — serving skilled nursing facilities in the Portland/Hillsboro market.

In this regional role, you’ll serve as the key connection between hospitals, families, and our facilities — helping patients transition to post-acute care with confidence and clarity.

You’ll play a critical role in coordinating seamless admissions, building trust with healthcare partners, and ensuring patients receive the care they need in a setting that supports healing and quality of life.

Key Responsibilities:

  • Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers

  • Evaluate patient referrals and assist with clinical screenings and pre-admission assessments

  • Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care

  • Serve as a clinical resource and liaison to families and referral sources

  • Collaborate with internal facility teams to align care plans and meet resident needs

  • Track referral activity and participate in census development initiatives

Qualifications:

  • Active LPN or RN license (preferred) or clinical background in healthcare, case management, or marketing

  • Experience in skilled nursing, post-acute, or hospital-based discharge planning preferred

  • Excellent communication, organization, and relationship-building skills

  • Ability to work independently and travel regularly between facilities

  • Must reside within commuting distance of Portland/Hillsboro, Oregon
