Job Summary

Own the admissions lifecycle for a Skilled Nursing facility, driving referrals, tours, and the end-to-end intake experience while building community partnerships to sustain census and compliance readiness.

  • Admissions Process Ownership: From referral through admission, including tours, paperwork, and onboarding support.
  • Community & Partnerships: Build relationships with hospitals, physicians, case managers, and community partners to generate referrals and raise the facility’s presence.
  • Performance Insights: Track satisfaction and outcomes feedback, surface trends, and partner with leadership to act on improvements.

Job Description

Responsibilities

  • Serve as the primary ambassador for the facility by building strong relationships with hospitals, physicians, case managers, and community partners
  • Coordinate and implement effective facility marketing and outreach programs to increase census and strengthen community presence
  • Conduct facility tours, manage admissions paperwork, and ensure a welcoming and seamless admission experience
  • Meet with prospective residents and families to educate, guide, and support them through the decision-making process
  • Lead and coordinate the full admissions process from referral to admission
  • Actively represent the facility in the community through networking, meetings, and events
  • Provide daily updates to the Admissions Support Team regarding marketing efforts, referrals, and admissions activity
  • Make informed admissions recommendations to facility leadership
  • Maintain working knowledge of Medicare, Medicaid, Managed Care, and applicable federal and state regulations
  • Monitor customer satisfaction feedback, analyze trends, and collaborate with leadership to develop action plans for improvement

Requirements

  • Prior experience in a Skilled Nursing care setting preferred
  • Demonstrated ability to understand, communicate, and promote clinical outcomes, data, and performance metrics to referral partners and the community
  • Proven experience with networking, community outreach, and coordinating community or referral-based events
  • Strong verbal and written communication skills
  • Bachelor’s degree required (or equivalent healthcare marketing or admissions experience)
