Job Summary

Own the on-site activity program across assisted living and long-term care facilities, acting as the hospice community ambassador and driving engagement, outreach, and reporting.

  • Community-facing activity planning and event coordination within facility networks.
  • Relationship management with facility staff, residents, and families to promote hospice services.
  • Tracking visits, activities, and outcomes for reporting and continuous improvement.

Job Description

Responsibilities

  • Schedule and coordinate activities and events with Assisted Living and Long-Term Care facilities.
  • Plan, organize, and conduct resident-focused activities, games, educational presentations, holiday events, and social gatherings.
  • Develop and maintain positive relationships with facility administrators, activity directors, nursing staff, residents, and families.
  • Represent the hospice organization professionally at all times.
  • Promote awareness of hospice services through community engagement and facility-based activities.
  • Assist with marketing initiatives and outreach efforts within assigned facilities.
  • Maintain activity calendars and coordinate event logistics.
  • Track facility visits, activities conducted, and outcomes for reporting purposes.
  • Distribute marketing materials and educational resources as appropriate.
  • Attend team meetings and participate in community events as requested.

Requirements

  • High school diploma or equivalent required; additional education in healthcare, marketing, recreation, or related field preferred.
  • Experience conducting activities in Assisted Living, Long-Term Care, Skilled Nursing, or similar healthcare facilities preferred.
  • Experience in healthcare marketing, community outreach, or customer service is a plus.
  • Excellent interpersonal, communication, and relationship-building skills.
  • Ability to engage and interact effectively with elderly individuals and their families.
  • Strong organizational and time-management skills.
  • Self-motivated with the ability to work independently.
  • Proficient in basic computer applications including email and Microsoft Office.
  • Valid driver's license, reliable transportation, and proof of insurance required.
  • Ability to travel regularly between facilities.
  • Ability to stand, walk, bend, and participate in activities for extended periods.
  • Ability to lift and carry activity supplies and marketing materials up to 25 pounds.

Preferred

  • Additional education in healthcare, marketing, recreation, or related field preferred.
  • Experience in healthcare marketing, community outreach, or customer service is a plus.
