Job Summary

Build and sustain a local referral network to expand access to Hope programs, focusing on channeling patients to Hospice, PACE, and related services while educating providers and communities to drive meaningful enrollment.

  • Develop and manage referral sources to meet access goals for assigned Hope programs.
  • Collaborate with healthcare providers and educate communities to support enrollment and appropriate admissions.
  • Translate community needs into coordinated outreach and education efforts to improve program uptake.

Job Description

Responsibilities

  • Increase access to care for the assigned Hope program (e.g., Hospice, PACE) by identifying and growing referral sources.
  • Meet organizational access-to-care goals by aligning referrals with community needs for Hope programs and services.
  • Use resources and educational techniques to enhance referrals.
  • Identify opportunities with other health care providers to support enrollment or appropriate admission into the assigned Hope program.
  • Provide community education when requested by supervisor.
  • Ensure referral sources choose a Hope program for their patients, such as Hospice, PACE, Palliative Care, Hope Choices, or Hope Bridges.

Requirements

  • Minimum 2 years of proven healthcare sales performance with a degree in marketing, business, public relations or related field, or healthcare background such as nursing, social work, or other patient-centered role.
  • Must have strong communication skills.
  • Previous sales performance training preferred.
  • Must possess a valid Florida Driver's License, current auto insurance, and reliable transportation.
  • RNs, Social Workers must keep all licensing current.
  • Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
