Job Summary

Own core referral-network development and cross-functional coordination to move patients through transitions while expanding the footprint in Versailles and Fairborn.

  • Territory ownership: build and track referral relationships, monitor trends, and surface growth opportunities.
  • Operational liaison: act as the central contact among providers, patients, families, and admissions teams to ensure smooth handoffs.
  • Growth and outreach: educate sources, participate in events, and maintain activity records to guide strategy.

Job Description

Responsibilities

  • Develop and maintain referral relationships within assigned territory
  • Serve as the primary liaison between healthcare providers, referral sources, patients, and families
  • Educate referral sources on services, programs, and admission criteria
  • Conduct routine visits to hospitals, physician offices, assisted living facilities, and community partners
  • Collaborate with clinical and admissions teams to ensure smooth patient transitions
  • Identify opportunities for business growth and community outreach
  • Participate in networking events, health fairs, and marketing initiatives
  • Maintain accurate records of referral activity and territory development efforts
  • Monitor market trends and competitor activity within assigned territory

Requirements

  • Active and unrestricted LPN or RN license in the State of Ohio
  • Previous experience in healthcare marketing, admissions, case management, or business development preferred
  • Strong understanding of healthcare systems and referral processes
  • Excellent communication, relationship-building, and presentation skills
  • Ability to work independently and manage territory effectively
  • Proficient with Microsoft Office and CRM/documentation systems
  • Valid driver’s license and reliable transportation required

Preferred

  • Experience working in skilled nursing, rehabilitation, home health, hospice, or long-term care
  • Established relationships within the Versailles and/or Fairborn healthcare communities
  • Strong organizational and problem-solving abilities
