Job Summary

Own regional hospice development by building referral networks, educating clinicians, and guiding patients from referral to admission while tracking progress against leadership goals.

  • Growth/Partner Ecosystem: manage relationships with hospitals, physicians, SNFs, discharge planners, and community partners.
  • Referral-to-admission Execution: coordinate events, educate stakeholders, and support conversations and consent with patients and families.
  • Market Intelligence: report weekly on successes, competitor insights, and growth opportunities.

Job Description

Responsibilities
  • Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners
  • Plan and coordinate marketing events, educational sessions, and community luncheons
  • Educate healthcare stakeholders on hospice and palliative care services
  • Support patients, families, and referral partners throughout the referral and admission process
  • Provide hospice informational / goals of care conversations with patients and families
  • Facilitate initial patient consent process
  • Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities
  • Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership
Requirements
  • LVN, RN, or MSW required
  • 1–2 years of healthcare marketing/sales experience preferred
  • Minimum of 2 years hospice experience highly preferred
  • Strong familiarity with local referral partners including:
  • Hospitals
  • Physicians
  • Board & Care (B&C)
  • Assisted Living Facilities (ALF)
  • Residential Care Facilities for the Elderly (RCFE)
  • Skilled Nursing Facilities (SNF)
  • Excellent written and verbal communication skills
  • Self-directed with the ability to work independently
