Build and maintain professional relationships with case managers, social workers, discharge planners, and community partners.

Provide timely follow-up on inquiries and keep leadership updated on referral opportunities.

Job Description

Conduct local community outreach and marketing
Visit hospitals, rehab centers, assisted living facilities, physician offices, and community organizations
Introduce agency services to healthcare professionals and referral sources
Build and maintain professional relationships with case managers, social workers, discharge planners, and community partners
Follow up on referrals and inquiries professionally and promptly
Assist with business development and community networking
Help distribute marketing materials and agency information
Provide updates on outreach activities and referral opportunities
Represent the agency professionally within the community
